If an eligible small employer has 5 employees, with salaries under $50,000, but only covers 1 under the company policy, and pays over 50% of the premium (other employees opted out of coverage) does the employer qualify for the credit on the one employee? Also, do all employees have to be included in the calculation of full time equivalent employees and average annual wages. I cannot find any examples by the IRS or the instructions that address this. This is so new, its still unchartered territory!!

Any information and/or resources to learn more will be very helpful.
